PROJECT: MEDICAL COMMERCIAL
LOCATION: LAKE FOREST or HIGHLAND PARK
ROLE: BASEBALL PLAYER
DATE: TUESDAY, 8/15
RATE: STANDARD SAG BG RATE
DESCRIPTION: MALES, AGES 9-12, ALL ETHNICITIES
***MUST BE SAG***
***MUST HAVE A VALID ILLINOIS WORK PERMIT***
If you are interested and available on 8/15, send an email with the following to commercials4extras@gmail.com:
1. Put “SAG MINOR” in the subject line of your email.
2. In the body of the email include your: name, age, height, weight, phone number, email, and current zip code.
3. Confirm that your child is SAG and include their SAG #.
4. Attach one current full body photo and one close-up. PLEASE RENAME YOUR PHOTOS.
5. Confirm your child has a valid IL work permit and attach a photo of it.
6. Confirm that you child has transportation to the locations listed above.
